Request for Issuing a Duplicate Passbook

Question

Sir, 
    Sorrowfully I want to bring in your knowledge that I am a regular customer of your branch and I have been maintaining my account for several years. But unfortunately yesterday I lost my passbook while going to school. My name is Rabi Roy and my account number is 123-456-789-11. I did report this to the bank already but I would like to follow up and see what more that can be done.
    I, therefore, request you to issue a new passbook for me. I shall be highly grateful to you if you issue me this at your earliest convenience.
